Submit a Nomination
To submit a nomination, you'll need to send us the following information:
- The name and address of your candidate, as well as your own name and address, for correspondence purposes.
- A short description of your nominee's specific deed or contribution to the community, including the scope and impact of the nominee’s work.
- The names and contact information of three (3) references that could support your nomination and maintain confidentiality.
There is no deadline for submissions. Nominations are accepted throughout the year.
All nominations are kept confidential to respect privacy and to avoid disappointment if the nominee is not selected. Nominators and others involved in the process are asked to respect this policy.
We are currently developing a new nomination tool that we are looking forward to launching in the new year. The new tool will make submitting nominations online quick, convenient and easy. In the meantime, however, please continue to use this PDF form.
